We have a few different ones that we use in quickbooks but I also have them in duplicate so I write invoices on the jobsite. We have one standard form and we have one with an area for the customer to enter credit card info and pay by credit card. It is in word, is editable, you can type right on the form and print it to make it look more professional, it will come to you with your business information already on it and it has a spot where the customer initials that the work is done to their satisfaction. It has a tear off pay stub to eliminate carbon copies. You also don't buy bulk quanities like at NEBS. You print what you need or you can take to a copy center and copy what you need there. If your phone number changes, you are not stuck with a bunch of useless forms. It's done a lot for us in supporting higher price points and making the client feel pampered and special. Steal the idea if you'd like.
